DIVERSE: A Mnemonic for Patient Encounters

This webpage provides a mnemonic to assist pysicians and other clinicians in the development a personalized care plan based on aspects of culture and diversity. Users of this tool should place it in the patient's chart or use the mnemonic when gathering the patient's history. The mnemonic also includes sample questions that the provider can ask patients.
