2008 In this document the fourteen grantees of the Robert Wood Johnson Foundation Diabetes Initiative tell their stories. The project provides self-management skill building for people with or at risk for diabetes and their caretakers through a culturally competent diabetes self-management course and related comprehensive services. This document provides an overview of the strategies implemented to reach rural, frontier and Indian Country settings, Latinos, African Americans, American Indians, and White populations, and the lessons learned from working with these populations.
